front wheel bearing

Can some one help me! My front wheel  is shaking, every time I turn the handle bar all the way to the right. I took the front wheel out and checked at the bearings, and they seem ok to me, but the I noticied that the spacer it's some kind of loose inside the wheel, is this normal?? if is not can you guys explain me step by step, how can I do to fix it.

The spacer on the Left side of the front wheel is not attached to the wheel or bearings, it simply slips over the axle as the axle is slid between the left fork and the front wheel. How do you know the front end shakes at full wheel lock. hopefully, your not doing any kind of speed at full wheel lock. If the shake occurs while you're rolling down the road, I would look more toward steering head bearings. Have you checked that the forks are straight? Is this a running bike that was not shaking prior, or a recently acquired bike that you're getting running?
